Use this application form to request additional time in which an association incorporated under the Associations Incorporation Act 2015 (the Act) is able to hold its Annual General Meeting (AGM).

This form should be lodged as soon as possible after the association becomes aware that it will be unable to comply with its obligations.

Notification of resignation of auditor or reviewer

This application form should be used to notify the Commissioner for Consumer Protection that an association incorporated under the Associations Incorporation Act 2015 (the Act) has received a resignation notice from its appointed auditor or reviewer.

WARNING: It is an OFFENCE pursuant to section 87(6) of the Act, with a penalty of $1,000, if an Association fails to notify the Commissioner within 14 days of receiving notice.

Notification of an incorporated association's address

This application form should be used to notify the Commissioner for Consumer Protection of the association’s address and postal address for service of notice under the Associations Incorporation Act 2015 (the Act).

WARNING: It is an offence to fail to lodge this notice either within 90 days of the new laws commencement or 28 days of the change occurring.

Certificate and statement of a management committee member (required for changing of rules)

Clubs, groups, organisations or associations use this form to provide a certification or statement that resolutions have been passed in accordance with the Associations Incorporation Act 2015.

This form is to accompany a relevant application, and is only required in the event that the person lodging the application is not a member of the association’s management committee.

Notice of special resolution to change rules

Use this application form to lodge the changes made to an incorporated association’s rules (also commonly known as the constitution) by special resolution of its members in accordance with the Associations Incorporation Act 2015 (the Act).

WARNING: It is an offence to fail to lodge within one month of the special resolution, unless the Commissioner for Consumer Protection has granted further time under section 30(3).

Deregistering an incorporated association

A benefit of incorporating is an association has perpetual succession, which means it can continue to exist regardless of any changes to it its membership.

However, there may come a time where the association has a lack of members or there is no longer any interest in continuing its activities and the incorporation of the association should be ended.

Ending an association

There are a number of ways in which an association’s incorporation can cease. These include the:
