This application form should be used to notify the Commissioner for Consumer Protection that an association incorporated under the Associations Incorporation Act 2015 (the Act) has received a resignation notice from its appointed auditor or reviewer.
WARNING: You must submit this form 14 days of receiving resignation notice. Failure to do so may incur a penalty.
