Use this application form to classify an incorporated association’s financial tier under the Associations Incorporation Act 2015 (the Act) as its usual size (Tier 1 or Tier 2) even if its revenue has changed due to an unusual and non-recurring event.
This form must be submitted during the financial reporting period in which the unusual event occurred or within three months of the financial reporting period ending.

Use this application form to request the Commissioner for Consumer Protection’s leave to be to be appointed or act as a member of a management committee of an incorporated association if you are otherwise prohibited from doing so under the Associations Incorporation Act 2015 (the Act).
This form must be lodged prior to an appointment being made.
Please note: You are only required to declare offences that occurred in the past five years from either the date of conviction or release (whichever is the latter).
This application form should be used by clubs, groups or associations previously incorporated under the Associations Incorporation Act 2015 (the Act) that have been cancelled and are seeking to have their incorporation reinstated.
