A recent coronial inquest was held into the death of a 22 month old boy, who tragically died at the family’s Yokine home in October 2013.  Forensic pathologists found that the boy had died of injuries suffered when the 1.25m tallboy he was climbing on toppled onto him.

During the inquest the boy’s mother gave evidence that before the tragedy she had asked her landlord to allow her to bolt the tallboy to a wall but her request had been denied.

Co-operative form 72 - Application for voluntary deregistration of a co-operative

This form is used to apply for voluntary deregistration of a co-operative under the Co-operatives Act 2009 (the Act) provided it meets certain criteria. Once deregistered, the co-operative ceases to exist as a legal entity and can no longer do anything in its own right.

Before applying co-operatives need to ensure that they meet the following requirements:

Co-operative form 01 - Application to approve co-operative’s rules and formation disclosure statement

This form must be submitted at least 35 days prior to the co-operative's formation meeting If needed, you can apply for, or a shorter timeframe using the Form 12 - Application for an extension or abridgement (shortening) of time.

What do I need to complete a form

To complete the submission of the form you must:

Co-operatives use this form to apply to the Registrar of Co-operatives to extend or abridge (shorten) a time limit for doing anything required under Co-operatives Act 2009 (the Act) or by the co-operatives rules.

NOTE: If an extension is required for more than one purpose, separate applications must be made.

To submit the return you, must lodge a complete form. This means you must:

This application must be lodged at least 21 days (or a shorter period the Registrar may allow in a particular case) before notice of the special resolution is to be given to members.
