Privacy collection notice

The personal information you provide in this form and in further correspondence relating to this complaint will be used by the Department of Local Government, Industry Regulation and Safety (LGIRS) to:

  • assess, manage and respond to your complaint,
  • seek your feedback on our service; and
  • maintain a database of complaints to monitor community trends.

We may provide this information to:

  • relevant government agencies or departments, another authority or third party, as required to respond to the matters raised in your complaint.
  • authorised internal staff to support appropriate follow up and administration of your complaint.

We will not disclose personal information about you to anybody else, unless you have given consent, or we are authorised or required to do so by law. 

Providing us with the requested information is voluntary. If you choose not to provide us with it, this will limit our ability to assess your complaint to determine how we can assist you.
