Starting a Co-operative

Form 01 Application for approval of co-operative's rules and formation disclouure statement

This form is used to obtain approval of the draft rules and draft formation disclosure statement (if necessary) prior to the formation meeting.

This form is used to register a co-operative following the formation meeting.

Changing the rule forms

Form 30 Co-operative Application for approval of proposed alteration to the rules

This form is used to obtain approval to amend rules concerning active membership provisions or conversion from a non-distributing co-operative to a distributing co-operative and vice versa.

Form 32 Co-operatives application for registration of an alteration of rules

This form is used to register a speacial resolution or an amendment to the rules of the co-operative.

This form is used by to lodge a co-operative's annual return to the Registrar.

Form 12 Co-operatives application for extension or abridgement of time

This form is used to obtain approval for an extension or shortening of specified time frames in which to undertake or complete required activities, including duties required under the co-pperative's rules.

Form 13 Co-operatives notice of appointment or cessation of directors and officers

This form is used to inform us when a secretary, chief executive officer or directoris appointed or leaves the co-operative.

This form is used to notify of a change to the co-operative’s registered office.

This form is used to notify when the co-operative's auditor resign, is removed or ceases to be the auditor of the auditor.

From 1 November 2023, we will no longer accept lodgement fees by credit card for co-operative forms submitted by post.

If you intend to pay by credit card, once we receive your form you will be issued with a Payment Number (PN) so that you can make payment using our secure online payment portal.

Alternatively you can take your completed form and pay by credit card in person at our Cannington office or any of our regional offices.
