Form 12

Use this application form to request the Commissioner for Consumer Protection’s leave to be to be appointed or act as a member of a management committee of an incorporated association if you are otherwise prohibited from doing so under the Associations Incorporation Act 2015 (the Act).

This form must be lodged prior to an appointment being made.

Please note: You are only required to declare offences that occurred in the past five years from either the date of conviction or release (whichever is the latter).

This application form should be used by clubs, groups or associations previously incorporated under the Associations Incorporation Act 2015 (the Act) that have been cancelled and are seeking to have their incorporation reinstated.

Form 1T

Use this application form to apply for approval to become registered as a different type of prescribed body corporate under another Act.

This form must be submitted to the Department of Commerce within 28 days of the special resolution to transfer being passed.

Form AUEXP

This application form should be used by an association incorporated to obtain an exemption from its obligation to distribute a copy of an auditor or reviewer’s representation to its members under the Associations Incorporation Act 2015 (the Act).

Form 18

Use this application form to apply to amalgamate two or more incorporated associations into a single new incorporated association under the Associations Incorporation Act 2015 (the Act).

Purpose

For use when an incorporated association passes a special resolution in accordance with the Associations Incorporation Act 2015 (the Act) to voluntarily cancel its incorporation. The incorporation of an association can only be voluntarily cancelled if it is solvent.

Form 3

Use this application form to request additional time in which an association incorporated under the Associations Incorporation Act 2015 (the Act) is able to hold its Annual General Meeting (AGM).

This form should be lodged as soon as possible after the association becomes aware that it will be unable to comply with its obligations.

Clubs, groups, organisations or associations use this form to become an incorporated association under the Associations Incorporation Act 2015 (the Act).
